Pre-Visit Client Questionnaire – Fill this out before your appointment!
The New Client Form is required to be signed once per year by all clients, not just new clients.
Late Policy:
Because we want to ensure that each pet scheduled gets their full, allotted time, we have instituted a late policy. If you are more than 10 minutes late to your appointment, we may have to have you drop off your pet to be seen the same day or we may have to reschedule. We don't want to cut your pet's time short, nor that of any other scheduled pet. We thank you in advance for your understanding.
Cancellation Policy:
If you need to cancel or reschedule your appointment, we kindly ask that you give us 24-hour notice, so we have time to schedule another patient. A No-Show fee of $25 for appointments or $100 for surgeries/dentals may be billed to your account if we do not receive at least 24 hours’ notice prior to cancellation of your appointment and may require a non-refundable deposit before any services are rendered in the future.
